Please note that the following Occupational Medicine procedures will only be offered daily until 7 pm:
Employment physicals (both pre-employment and annual)
Pre-employment drug screens (both regulated and non-regulated)
All other OccMed services
A few important exceptions:
After 6 pm, we will continue to perform all post-accident, reasonable suspicion, and random drug/alcohol screens. In addition, we will continue to perform Workers’ Comp injury care. We apologize for any inconvenience.
Pre visit
Bring your photo ID (driver's license or state ID). CDL holders must bring their driver’s license
Medical Authorization Form. Your employer should have provided one to you.
Company-specific paperwork that needs to be completed by the provider
During visit
Provider visits: Our experienced providers will attentively listen to your symptoms and medical history and perform a comprehensive exam to ensure that you can be safely placed in your job or appropriate treatment of your work-related injury.
Non-provider visits: Our clinicians will perform all required testing in a timely and efficient manner. Keeping you informed of the process.
Post visit
Secure Communication: placement recommendations and appropriate test results will be sent to your employer in an encrypted, secure method.
Follow-up: if a re-check appointment is necessary, one will be scheduled for you before you leave the clinic
All necessary treatment paperwork and work status notes will be provided to you at discharge that you can share with your employer.
Your employer will also receive a copy of the records that apply to your injury.
No. When you set up an account with WellNow Urgent Care, same-day reporting will be custom to your request. We can send results via fax or email.
Organizations do not need a service agreement in place prior to obtaining occupational medicine services for employees. However, we do encourage companies to establish service agreements with us as they help to delineate which services your employees need as well as streamline communication for billing and reporting. With a service agreement in place, we can provide follow-up occupational medicine services for workers’ compensation cases. For walk-in occupational medicine patients whose employers do not have service agreements, payment is required at the time of service.
Yes, our providers in all of our locations are on the National Registry list.
